Work Schedule : 100% FTE, days. Standard schedule would be between the hours of 8:00 am - 5:00 pm Monday through Friday. You may be eligible for up to a $3000 sign-on bonus. No on call or weekend requirements. This is a hybrid position. When in person, you will work at 705 S. University Ave in Beaver Dam, WI.

Be part of something remarkable This position is part of the Collaborative Care workstream in Primary Care. As a member of the Collaborative Care team, you will work as a team with the primary care staff and team psychiatrist helping deliver brief interventions for patients in Primary Care.
We are seeking a
Behavioral Health Therapist or Clinical Psychologist to:
- Collaborate with other medical professionals in a fast-paced environment.
- Provide brief evidence-based interventions.
- Provide short, but intense follow-up.
- Connect patients to resources.
- Use outcome-based models to track patient improvement.

If Behavioral Health Therapist: Education: Minimum - Master's Degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or other behavioral health related field
Work Experience: Minimum - 2 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults
Preferred - 5 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults
Licenses and Certifications: - Licensed Clinical Social Worker Upon Hire Required or
- Licensed Professional Counselor Upon Hire Required or
- Licensed Marriage and Family Therapist Upon Hire Required and
- Basic Life Support/CPR within 6 months Required and
- Substance Abuse Counselor Upon Hire Preferred or
- Clinical Substance Abuse Counselor Upon Hire Preferred
If Clinical Psychologist: Education: Minimum - Doctor of Philosophy (Ph.D.) or Psychology (Psy.D.) in Counseling or Clinical Psychology or related field.
Work Experience: Minimum - One (1) year of postdoctoral supervised clinical experience
Licenses and Certifications: Minimum -
- Licensure or eligible for licensure as a Psychologist in Wisconsin
- Basic Life Support/CPR
