Principle duties of this role is to provide clinical mentorship to ensure plan of care accuracy, utilization management and timely submission of documentation to ensure success in the new Medicare payment structure - Patient Driven Groupings Model (PDGM). ChristianaCare HomeHealth is seeking a Full Time RN to work in either our Smyrna or Avenue North office providing mentorship to clinicians to ensure timely and accurate plan of care development and maintenance. The successful candidates will work Monday through Friday between 9am-5:30pm; and a weekend rotation - usually 1 out of 7. May have the opportunity after orientation to work the weekend commitment remotely.
