As the Case Management Nurse Coordinator, this person will oversee the growth and maintenance of the care coordination program. The RN Coordinator will provide oversight and management of enrolled patients. They will also ensure the completion of the annual wellness visit and follow up on all elements of the preventative plan of care. The Care Management RN Coordinator will also collaborate with intra and interdepartmental staff of the organization, outreach, disease management, health promotion, and education/training.
