Job Description

Hospice of Lake Cumberland provides quality care for patients and their families in their home and facilities within our seven county service area of Pulaski, Russell, Casey, McCreary, Wayne, Clinton and Cumberland counties. This person will cover a designated area but may have to travel to any of our seven counties.

This will be a part-time schedule. There is potential for on call responsibilities so there is some night and weekend work when you are scheduled for on call. You receive an hourly stipend for being on call and if you have to go out you get paid hourly in addition to your annual salary. Job Title: Chaplain

Job Summary

Provides spiritual care and support to patients/families in accordance with their personal desire and belief system. Chaplain will also endeavor to create and nurture a spiritual presence within the Hospice interdisciplinary team and care environments.

Qualifications

  • Graduate from a school of theology or accredited college or university with a BA degree in Religion, Philosophy, Pastoral Studies or Counseling.
  • Two years experience in pastoral ministry or in a spiritual chaplaincy position.
  • Competency in community relations, interpersonal relationships, and oral and written communication.
  • Ability to provide spiritual care within the Hospice concept or terminal care without judgment of others’ personal spiritual beliefs.

  • Supervised by Director of Clinical Services

Duties and Responsibilities

Provides spiritual care and support that is responsive to the varying spiritual needs of Hospice patients and families, and to the greater Hospice community.

  • Provides spiritual assessment, counseling and support to patients and families.
  • Participates in the preparation, evaluation and execution of the interdisciplinary care plan to assure continuity of care and services.
  • Works collaboratively as a member of the Interdisciplinary Team.
  • Encourages existing denominational affiliations; facilitates involvement of patient’s own pastor as desired by patient and family.
  • Provides direct spiritual counsel and/or support to patients and families who have no religious affiliation or request services.
  • Maintains timely and accurate documentation of all spiritual care provided by staff and community clergy.
  • Contributes to ongoing staff and volunteer education to maintain team awareness of spiritual needs and to further develop spiritual assessment skills.
  • Provides spiritual and emotional support to hospice staff members and clergy working with hospice patients.
  • Serves as a staff resource regarding religious beliefs, practices and rituals.
  • Performs occasional liturgical assignments such as memorial services and funerals.
  • Provides guidance in the ethics of medical care decisions.
  • May be required to work in the inpatient unit.

Assist in the provision of bereavement services for Hospice patients and families as well as the community.

  Facilitate community bereavement groups as needed.

  • Assist with Hospice memorial service.
  • Participate with Hospice bereavement camp for children.
  • Perform bereavement counseling for the community and assist community in focusing on death and dying issues in their community.

Contributes to the overall success of Hospice by cooperating in other organizational requirements and activities.

  • Communicates information concerning work schedule and need for time off to supervisor.
  • Participates with on-call schedule to provide twenty-four hour, seven day a week coverage.
  • Participates in Hospice of Lake Cumberland’s orientation and in-service training programs for professional and volunteer staff.
  • Participates in organizational committees and special projects as assigned.
  • Participates in community programs as requested to promote better understanding of Hospice.
  • Participates in professional liaisons with other clergy and pastoral and health care providers.
  • Performs other duties as may be assigned.
