Job Description

Salary Range:  $45.4100 - $69.8500 /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.

 

Primary Duties and Responsibilities

The Faith Community Outreach RN (FCN) works within a specific area of Hoag’s Spiritual Care Department serving as a spiritually-based health resource to the targeted community while promoting optimal bio, psycho-social, cultural and/or environmental dimensions of health. The FCN Outreach Nurse will develop and coordinate programs/projects designed to meet client needs while promoting growth of the Hoag FCN Program within the faith community & throughout the Hoag service area. 

A Faith Community Nurse provides spiritually based care and outreach in a congregational setting and/or administrative support in a Hoag office setting serving as a point person for other FCNs. The FCN uses population health concepts to identify specific client, group or community needs, coordinates resources, and provides necessary services to clients, such as health education, screenings, health counseling, advocacy, developing support groups, etc. Oversees & organizes Flu Vaccination clinics including giving shots, and other community educational or service events. Maintains required documentation and regulatory compliance standards.

The FCN provides administrative and development support to respective program including develop new programs, policies & procedures, secure CEU approval, schedule events, analyze program costs & stay within budget for program administration, evaluate participation/ satisfaction of programs, compile data and reports, and other administrative and oversight aspects of community-based and education projects.

Interacts with people of many faith traditions. May require frequent weekends and some evenings. Serves as a Hoag Ambassador to the FCNs, clergy, visitors, staff, vendors, and the community. Performs other duties as assigned. 

 

Education and Experience

Required : Proficiency in Microsoft Office required. Experience working with diverse clients including a variety of faith, ages, ethnicity, socio-economic, and health issues required.

Preferred: BSN, Master’s, PHN Certificate preferred. Bilingual preferred for certain assignments.

Additional requirements vary by specialty area and are as follows:

Faith Community Outreach: Two (2) years’ experience in nursing or equivalent public health experience required. Must complete Faith Community Nurse Foundations Course within one (1) year of hire. A personal faith experience to understand and speak about spirituality. One year of experience as a Faith Community Nurse (FCN) preferred.

 

License Required

Current Registered Nurse (RN) in the state of California

Current CA Driver’s license

 

Certifications Required

BLS - AHA Basic Life Support
