We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our dedicated team, focusing on providing exceptional care to clients served by the Department of Behavioral Health and Developmental Disabilities (DBHDD). The LPN will play a vital role in delivering personalized and client-centered healthcare, ensuring the well-being and comfort of our clients in collaboration with a multidisciplinary team.  Patient needs help with transportation to doctor visits, communication system, sugar level checks, and nail care. 

Responsibilities :

Client Assessment and Care Planning :

  • Conduct thorough assessments of clients' physical and mental health. 
  • Collaborate with healthcare professionals to develop individualized care plans. 
  • Monitor and update care plans as needed to address the evolving needs of clients. 


Medication Administration and Monitoring

  • Administer medications as prescribed by healthcare providers. 
  • LPN for Total Care, family gives meds, G-tube fed, occasional PRN meds for spasms.
  • Monitor and document the effects of medications, making adjustments as necessary. 
  • Educate clients and their families on medication management and potential side effects. 

Clinical Documentation :

  • Maintain accurate and up-to-date client records, ensuring compliance with regulatory standards. 
  • Document all interactions, observations, and interventions in a timely manner. 

Collaboration with Multidisciplinary Team :

  • Work closely with physicians, psychiatrists, social workers, and other healthcare professionals to coordinate comprehensive care. 
  • Participate in team meetings to discuss client progress, challenges, and adjustments to care plans. 

Client and Family Education :

  • Provide education to clients and their families on health conditions, treatment plans, and self-care strategies. 
  • Foster a supportive and informative environment to empower clients in managing their health. 

Qualifications :

  • Current and valid LPN license in the state of GA. 
  • Experience in nursing, preferably in behavioral health or developmental disabilities settings. 
  • Strong understanding of DBHDD regulations and guidelines. 
  • Excellent communication and interpersonal skills. 
  • Ability to work collaboratively within a multidisciplinary team. 
  • Compassionate and patient-focused approach to care.
