Summary The Nurse Case Manager is responsible for overseeing the health and medical care of individuals supported by Bello Machre. This role involves coordinating medical services, developing health plans, and ensuring compliance with all healthcare regulations. The ideal candidate is compassionate, detail-oriented, and committed to enhancing the well-being of individuals with developmental disabilities.
This is a full-time position, 40 hours per week, Monday through Friday. Hours are flexible and should be scheduled to effectively fulfill the responsibilities of the position and meet with supervisor’s approval.

Education And/or Experience - Graduate from an accredited nursing program and active RN or BSN license in Maryland (MD).
- Minimum of 2 years of nursing experience.
- Excellent management, organizational, problem-solving, communication and interpersonal skills.
Trainings, Licenses, and Certifications - Must have a valid and unencumbered Maryland Registered Nurse license in good standing.
- Must have a valid State Issued driver’s license. They may not have any not have more than five points at any time and may not have any alcohol and/or drug related citation on their license.
- Must successfully complete required training upon hire that includes but is not limited to include Bello Machre New Hire Orientation, Developmental Disabilities Orientation, First Aid, CPR, MANDT, Choking Prevention & Dysphasia Diets, Skills Training, Combating Constipation, RNCAMD, HRST Rater, HRST Reviewer, iCareManager (iCM), Supervisory trainings, and any additional required training as determined by the State of Maryland and/or Bello Machre.

Physical Demands While performing the duties of this job, the employee is regularly required to talk, hear, stand, walk, sit, bend, lift, push, pull, and reach with hands and arms.
Environment While performing the duties of this job, the employee will be in a hybrid office and remote setting, schedule subject to supervisor’s approval.

Medical Care Coordination - Develop, implement, and monitor individualized health care and care management plans based on client needs.
- Coordinate routine, preventive, and emergency medical appointments, ensuring timely follow-up.
- Conduct health assessments, identify risks, and provide appropriate nursing interventions.
- Respond promptly to medical emergencies and coordinate necessary care with healthcare providers.
Health Oversight & Compliance - Ensure adherence to healthcare protocols, policies, and state and federal regulatory standards.
- Monitor compliance with medication administration procedures and maintain up-to-date medical records.
- Maintain accurate client documentation, including care plans, assessments, medication logs, and progress notes.
- Review medication in various classes, controlled substances, disposal, count sheets, etc.
- Conduct regular health assessments and wellness checks to evaluate client well-being.
Training & Support - Train and mentor direct support staff on healthcare procedures, medication administration, and emergency protocols.
- Provide on-call support and after-hours assistance for health-related emergencies as needed.
- Participate in professional development sessions, mandatory trainings, and team meetings to stay current on best practices and policies.
Advocacy & Collaboration - Advocate for individuals’ health-related needs by ensuring access to quality medical care and services.
- Facilitate communication among healthcare providers, support staff, clients, and families.
- Collaborate with interdisciplinary teams, including case managers, program coordinators, and external providers, to optimize care and support.
- Support individual’ participation in community life while promoting their health, well-being, and independence.
Administrative & Professional Responsibilities - Maintain confidentiality and uphold ethical standards in all aspects of individual’s care.
- Participate in on-call rotations and provide after-hours support as required, i.e. weeknights, weekends, holiday coverage, etc.
- Attend and contribute to meetings, trainings, and professional development sessions.
- Perform other duties as assigned.
